Breast Lift Surgery vs. Breast Augmentation: Which Is Right for You?
A breast lift is a surgical procedure focused on raising and reshaping the breasts when they have sagged or lost firmness. It involves removing excess skin, tightening the surrounding tissue, and repositioning the nipple-areola complex to a more youthful height.
This surgery is ideal for women who are satisfied with their breast volume but unhappy with the way their breasts sit or slope due to aging, weight loss, pregnancy, or genetic factors. A lift can significantly improve the contour and overall silhouette without necessarily changing cup size. However, it does not add volume, which is a common misconception.
Breast augmentation is focused on increasing breast size and enhancing shape through the insertion of implants (saline or silicone) or, in select cases, fat transfer. This procedure is particularly suited for individuals seeking fuller breasts, enhanced cleavage, or correction of volume asymmetry.
While augmentation can slightly lift the breasts in cases of mild sagging, it is not effective for significant droop. Patients with low nipple placement or stretched skin may not benefit from implants alone and may require a lift in combination.
If your breast size feels appropriate, but the breasts have dropped, lost firmness, or sit low on the chest wall, a lift is often the right choice.
If your breasts still sit relatively high and shaped well, but you wish they were larger or had more volume and cleavage, augmentation alone may be appropriate.
If you have both volume loss and sagging — as is common after pregnancy or significant weight loss — a combination of both may be necessary to achieve your desired look.
We assess multiple clinical markers during consultation. One major factor is the nipple-areola position relative to the breast crease (inframammary fold). If the nipple is below this fold or points downward, sagging is significant, and a lift is likely required.
Another key factor is skin elasticity and existing breast tissue. If you have adequate volume but poor skin tone, we may focus on tightening and repositioning rather than enlargement. Conversely, if your tissue is thin but you want more projection, implants may be prioritized.
Future plans matter too. If you are expecting more pregnancies or anticipate major weight changes, we may advise delaying surgery or staging procedures to maintain long-term results.
When patients want fuller, perkier breasts with enhanced upper-pole volume, a combined procedure is often the most effective. However, combining lift and augmentation requires advanced surgical planning, as it involves managing two opposing forces: skin tightening and volume expansion.
Both procedures are typically performed under general anesthesia in our fully equipped surgical suites in Seoul. A breast augmentation alone may take about an hour, while a lift, especially with significant tissue reshaping, can take longer.
Postoperative recovery varies, but most patients can return to light activity within a week. A compression garment or surgical bra is worn for several weeks to support healing. Strenuous activities, including upper-body workouts, are restricted for 4 to 6 weeks.
The extent of scarring differs between the two procedures. Augmentation usually involves a smaller incision, typically under the breast (inframammary), around the nipple (periareolar), or less commonly, through the armpit (transaxillary).
In contrast, a breast lift involves more extensive incisions. Depending on the degree of lift required, the incisions may encircle the areola, run vertically down to the breast crease, and occasionally include a horizontal line along the fold. While scarring is unavoidable, it typically fades with time and can be minimized with laser treatments, silicone dressings, and proper aftercare.
Implants are not lifetime devices. While modern silicone and saline implants are durable, they may require replacement or revision after 10 to 15 years due to rupture, capsular contracture, or changes in aesthetic preference.
A breast lift tends to offer more long-term stability, especially when sagging is the primary concern. However, natural aging, gravity, hormonal changes, and lifestyle factors can still affect the results over time.
Breast augmentation is tailored accordingly. For patients with a smaller frame, narrow chest width, or thin skin, choosing an implant that harmonizes with the rest of the body is critical. Similarly, for those seeking a lift, maintaining a soft, natural contour is key. Overcorrection or excessive tightness is avoided.
Our surgeons apply the same artistic principles used in facial surgeries like eyelid and forehead procedures to body contouring. This means we treat each breast surgery as a sculptural enhancement — not simply a technical operation. This attention to proportion, symmetry, and ethnic-specific anatomy distinguishes our approach.
We also see a significant number of revision cases. Patients who have had surgery abroad or at other clinics often come to us for correction of poor scarring, asymmetry, or dissatisfaction with shape. Revision planning is complex, and understanding whether a lift, an implant, or both are needed is part of our specialized expertise.

You are less focused on size and more on shape and positioning
You have had children or experienced major weight changes, leading to stretched skin
Your breast shape and nipple position are acceptable, but you want more volume or enhanced cleavage
You want to correct volume asymmetry or increase your cup size
Your skin is firm and elastic enough to support an implant without significant droop
You are seeking enhancement rather than correction
You have both sagging and volume loss
You want a fuller breast shape and higher nipple position
You understand the complexity and are willing to commit to a longer recovery
You are in good general health and have realistic expectations

Ask yourself:
What bothers me most — the size of my breasts or their position?
Am I comfortable with my current volume if the shape could be improved?
Do I have sagging skin or low nipples that implants alone won’t fix?
What is my broader aesthetic goal, considering my body frame and cultural preferences?
