Zygomatic Reduction for Balanced, Beautiful Cheeks
At SoonPlus Plastic Surgery in Seoul, we often meet patients who feel their cheekbones give their face a wider or harsher appearance than they'd like. For these individuals, zygomatic reduction isn't about conforming to a standard of beauty—it's about creating facial harmony and balance. This procedure can be a life-changing step toward a softer, more proportional look that enhances the natural beauty of each patient.
Zygomatic reduction, commonly called cheekbone reduction or malarplasty, is a surgical procedure that reduces the prominence of the zygomatic bones—the paired bones that define the width of your midface. Prominent zygomas can create a strong, angular profile. While some individuals may view these features as striking or bold, others feel it overpowers their other facial features or makes them appear older or more severe.
To be honest, many of our patients come in not fully certain if they "need" zygomatic reduction. What they do know is that something feels out of balance in their facial proportions.
In Korea and many East Asian cultures, there's a strong aesthetic preference for the so-called "egg-shaped" or V-line face—a contour that softly narrows from the cheekbones to the chin. When the cheekbones protrude too much, especially in profile or three-quarter view, the face can appear flat or too wide.
However, this isn't purely about trends. Many patients simply want their external appearance to better match how they feel inside—more elegant, refined, or youthful. Others seek zygomatic reduction to correct asymmetry, minimize harsh shadows under the cheeks, or soften an overly dominant midface.
Common motivations include:
Reducing facial width for better balance
Creating a more feminine or softer profile
Correcting asymmetrical or uneven cheekbones
Enhancing facial harmony, particularly in combination with jaw or chin surgery
Boosting self-confidence in social and professional environments
At SoonPlus, we approach zygomatic reduction as both a science and an art. Each case requires an individualized plan. During surgery, our goal is not just to reduce bone, but to reposition and stabilize it in a way that preserves structural integrity and aesthetic grace.
Most zygomatic reductions are performed via intraoral incisions—inside the mouth—which means no visible scarring. In some cases, especially where the upper zygomatic arch is involved, a small incision near the temple hairline may be added to improve access.
We employ techniques such as the L-shaped or arched osteotomy to release the zygomatic body and arch. The bone is then carefully repositioned inward and slightly downward to reduce projection and facial width. The amount and direction of movement are precisely calculated to ensure both aesthetic and functional outcomes.
For more dramatic changes, a High-L osteotomy may be used. This allows for greater inward rotation and repositioning of the cheekbone structure, ideal for patients with significantly wide midfaces.
Once repositioned, the bones are secured using titanium plates and screws. We often use a four-point fixation method, which ensures a stable, even result and minimizes postoperative complications such as bone shifting or facial asymmetry.
Recovery from zygomatic reduction is more comfortable than many patients expect. Most can return to normal, non-strenuous activities within one to two weeks. Swelling and bruising are most noticeable in the first 7–10 days, but significant swelling typically subsides within a month. Full bone healing takes several months, during which subtle refinements in facial contour will continue to emerge.
We provide each patient with a detailed aftercare plan, which includes:
Sleeping with the head elevated
Using cold compresses to reduce swelling
Adhering to a soft or liquid diet during the initial healing phase
Oral hygiene instructions to prevent infection from intraoral incisions
As with any surgical procedure, zygomatic reduction carries potential risks. These may include:
Infection
Temporary or permanent numbness due to nerve irritation
Incomplete bone healing or malunion
Asymmetry or dissatisfaction with the final result
The good news? Most complications are rare when surgery is performed by a skilled, experienced team. Our surgeons have performed hundreds of facial bone contouring procedures, and we prioritize conservative techniques and meticulous planning to minimize risk. Patient safety and satisfaction are always our highest priorities.
Ideal candidates for zygomatic reduction are individuals who:
Have prominent or asymmetric cheekbones affecting facial harmony
Are in good overall health and free of untreated medical conditions
Have realistic expectations about the outcome
Understand the recovery process and are willing to follow aftercare instructions
We often find that patients undergoing other facial contouring procedures (such as V-line surgery or chin augmentation) benefit from combining these with zygomatic reduction for a more holistic and balanced result.
Our approach includes:
3D CT imaging for precise preoperative planning
Detailed facial analysis to customize your procedure
Multilingual consultations and seamless support for international patients
FDA-approved materials and state-of-the-art facilities
